健脾通络方治疗儿童代谢相关脂肪性肝病的临床研究
Clinical study on the treatment of pediatric metabolic-associated fatty liver disease with Jianpi Tongluo decoction

Abstract
Objective:Based on the theory of liver collateral disease,this study utilizes a self-formulated Jianpi Tongluo decoction to intervene in pediatric metabolic-associated fatty liver disease(MAFLD)cases,aiming to assess its effect on liver protection and effects on glucose and lipid metabolism.Methods:This study collected data from pediatric patients diagnosed with MAFLD at the Pediatric Liver Disease Clinic and inpatient department of Beijing Children's Hospital from January 2023 to December 2023.Patients were randomly assigned to a Western medicine control group or a Traditional Chinese Medicine(TCM)treatment group.The control group received lifestyle modification and liver-protective therapy with Glucurolactone tablets,while the TCM group was treated with Jianpi Tongluo decoction alongside lifestyle improvements.Laboratory markers associated with MAFLD and TCM syndromic scores were collected before treatment and after 8 weeks to evaluate the clinical efficacy of TCM treatment in children with MAFLD.Results:After eight weeks of treatment,the comparison between the TCM group and the control group indicated the following outcomes:a.Improvements in weight,Body mass index(BMI),and body composition:children treated with Jianpi Tongluo decoction showed significantly greater reductions in weight,BMI,and body fat,along with improved basal metabolic rate compared to the control group(weight:P=0.002;BMI:P=0.003;body fat:P=0.002;basal metabolic rate:P=0.002);b.Improvement in liver function markers:children in the TCM treatment group demonstrated significant improvements in alanine aminotransferase(ALT),menthyl amino acid transferase(AST),and glutamine transpeptidase(GGT)levels(ALT:P=0.001;AST:P<0.001;GGT:P=0.039);c.Positive changes in glucose and lipid metabolism:the TCM treatment group exhibited reductions in total cholesterol(TC),low-density lipoprotein cholesterol(LDL),fasting insulin(FINS),and insulin resistance index(HOMA-IR)levels and an increase in high-density lipoprotein cholesterol(HDL)levels(TC:P<0.001;LDL:P=0.006;FINS:P=0.044;HOMA-IR:P=0.048;HDL:P=0.005),with the TCM group outperforming the control group in reducing TC levels(P<0.001).d.Reduction in TCM syndromic scores:significant reductions in TCM syndromic scores were observed in the treatment group,indicating notable symptomatic improvement(P<0.001).Conclusion:Jianpi Tongluo decoction demonstrates marked therapeutic efficacy in treating pediatric MAFLD,effectively improving weight,BMI,liver function markers,lipid levels(TC),FINSulin resistance,and body composition(reducing body fat and increasing basal metabolism).Additionally,the treatment significantly reduces TCM syndromic scores in children with MAFLD.This comprehensive efficacy underscores the potential advantages of Jianpi Tongluo decoction in enhancing basal metabolism and improving metabolic status,providing a foundation for integrated TCM and Western medicine approaches in the treatment and management of MAFLD.关键词
