PNPLA3基因多态性及其蛋白表达与乙型肝炎相关性肝癌的相关性及其对患者预后的影响
Correlation of PNPLA3 gene polymorphism and its protein expression level with hepatitis B-associated hepatocellular carcinoma and its effect on patients'prognosis

Abstract
Objective:Exploring the relationship between patatin-like phospholipase domain containing 3(PNPLA3)gene polymorphisms,its protein expression levels,and hepatitis B virus(HBV)-related liver cancer,along with their impact on patient prognosis.Methods:A total of 120 cases of chronic hepatitis B cirrhosis with cancer and 60 cases of chronic hepatitis B cirrhosis admitted to our hospital from November 2021 to June 2022 were selected as the hepatocellular carcinoma group and cirrhosis group,respectively.Sixty patients who underwent health checkups in our hospital during the same period were selected as the healthy control group.Differences in I148M typing of the PNPLA3 gene were detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-RCLP).The PPP2R3A protein expression levels in diseased liver tissues of patients in the hepatocellular carcinoma group and the cirrhosis group were compared,and the hepatocellular carcinoma patients were further divided into a high expression group and a low expression group.And the clinical characteristics of the two groups were analyzed.Kaplan-Meier curves and Log-Rank were used to test the survival differences between patients in the high-expression and low-expression hepatocellular carcinoma groups.Results:The difference in PNPLA3 gene I148M typing among the three study groups was statistically significant(P<0.05).The proportion of patients in the hepatocellular carcinoma group carrying the G allele was significantly higher than that in the cirrhosis group(χ2=4.364,P=0.037)and the healthy control group(χ2=17.686,P<0.001).The proportion of patients in the cirrhosis group carrying the G allele was significantly higher than that in the healthy control group(χ2=4.193,P=0.041).The rate of high expression of PNPLA3 protein in diseased liver tissues of patients in the hepatocellular carcinoma group was significantly higher than that in the cirrhosis group(59.2%vs 35.0%,χ2=9.349,P=0.002).In hepatocellular carcinoma patients,the proportion of patients carrying the G allele at the I148M locus of the PNPLA3 gene,HBV DNA positivity,and serum AFP,AFP-L3,and DCP levels in the group with high expression of PNPLA3 protein were significantly higher than those in the group with low expression(P<0.05).And the survival rate of the former group was significantly lower than that of the latter group during the follow-up period(Log-rank P=0.014).Conclusion:Mutation of I148M locus of PNPLA3 gene not only increased the risk of hepatitis B cirrhosis and hepatocellular carcinoma,but also the change of protein level caused by the mutation of this gene may have a close relationship with the disease progression and prognosis of hepatocellular carcinoma patients.关键词
