The Effect of Dual-Task Walking on Inter-Joint Coordination in Healthy Elderly Individuals and Healthy Young Adults

Abstract
Objective:To investigate the effects of dual-task walking on inter-joint coordination and coordination stability of the domi-nant lower limb in healthy elderly individuals and young adults,to identify the differences in motor coordination strategies between healthy elderly individuals and young adults,and to provide a theoretical basis for developing fall prevention rehabilitation programs for the elderly.Methods:The three-dimensional motion analysis system was utilized to collect spatiotemporal and kinematic param-eters during dual-task walking in the young group(aged 20~29 years,n=21)and the elderly group(aged 60~69 years,n=25).The continuous relative phase method was applied to calculate inter-joint coordination and coordination stability of the dominant lower limb's hip-knee and knee-ankle joints across five phases of the gait cycle.Results:The elderly group and the young group exhibited statistically significant differences in the center of mass velocity across all five phases of the gait cycle during dual-task walking(P<0.05).The elderly group showed decreased in hip-knee joint coordination stability during the first phase of the gait cycle(P<0.05),while no significant differences were observed in hip-knee inter-joint coordination and coordination stability in the remaining phases between the young and elderly groups during dual-task walking(P>0.05).The young group demonstrated enhanced knee-ankle inter-joint coordination during the second and third phases of the gait cycle(P<0.05),and reduced coordination stability during the third phase(P<0.05).The elderly group exhibited increased knee-ankle inter-joint coordination during the fourth phase of the gait cycle(P<0.05).No significant differences were found in hip-knee inter-joint coordination and coordination stability in the remaining phases between the young and elderly groups during dual-task walking(P>0.05).Conclusion:Healthy elderly individuals and young adults adopt different motor coordination strategies during dual-task walking.The young group demonstrated better hip-knee and knee-ankle inter-joint coordination during the stance phase of dual-task walking,while the elderly group exhibited better knee-ankle inter-joint coordination during the swing phase of dual-task walking.These results indicate that dual-task walking presents a challenge to inter-joint coordination in the swing phase for the elderly,providing a theoretical reference for designing lower limb inter-joint coor-dination training programs for the elderly population.关键词
