慢性肌骨疼痛患者身体、活动及参与功能对其疼痛程度的影响
Impact of Physical,Activity,and Social Participation Functions on Pain Intensity in Chronic Musculoskeletal Pain Patients:An ICF-RS-Based Study

Abstract
Objective:Utilizing the national standard"International Classification of Functioning,Disability and Health-Rating Set(ICF-RS)"to assess the functional status of outpatient patients with chronic pain in the neck,shoulder,waist,and leg regions,and further explores the impact of patients'functional levels,including their physical health,daily activities,and social participation,on their pain intensity.Methods:A total of 188 outpatient patients with chronic musculoskeletal pain were selected for general data col-lection and ICF-RS assessment.(1)General data:including basic demographic information(age,gender,height,weight,education level,occupation),whether using analgesics,past medical history(hypertension,hyperlipidemia,diabetes),and unhealthy lifestyle habits(smoking,drinking),etc.;(2)ICF-RS:used to assess the functional status of patients.Considering the specificity of the ICF-RS item threshold values and the characteristics of patients in this study,a binary classification method was adopted,converting dif-ferent threshold value results into the presence or absence of functional impairment(0-normal,1-abnormal);(3)Pain scoring method(NRS)quantifies patients'pain scores,and results are classified into 0～4 levels based on NRS scores,including normal,mild,moder-ate,severe,and extremely severe pain.An ordered logistic regression model was used to analyze the impact of patients'physical,daily activity,and social participation functional status on pain levels in patients with chronic pain in the neck,shoulder,waist,and legs.Results:Multivariable ordered Logistic regression analysis revealed that:(1)patient physical functions such as sleep function(OR=1.93,95%CI:1.033-3.608,P=0.039)and joint mobility(OR=3.214,95%CI:1.309-7.890,P=0.011)are independent factors af-fecting the pain level of outpatient chronic pain patients;(2)social participation functions such as entertainment and leisure(OR=2.089,95%CI:1.090-4.006,P=0.027)are independent factors affecting the pain level of outpatient chronic pain patients;(3)physi-cal functions(motor and endurance,muscle strength),daily activity functions(changing basic body posture,maintaining a body pos-ture,moving around,doing household chores,taking care of personal health),and social participation functions(using transporta-tion,paid employment)are synergistic factors affecting outpatient chronic pain patients.Conclusion:The physical,activity,and so-cial participation functions of outpatient patients with chronic pain can all affect their pain levels,which requires the attention of both patients and medical professionals.关键词
