Comparison of pregnancy and perinatal outcomes between fresh and frozen embryo transfers in patients with adenomyosis and decreased ovarian reserve

Abstract
Objective:To compare the pregnancy and perinatal outcomes of fresh em-bryo transfer between freeze-all and frozen-thawed embryo transfer(FET)in patients with ade-nomyosis and decreased ovarian reserve(DOR),and to optimize the application strategy of as-sisted reproductive technology.Methods:The clinical data of 446 patients who received in vitro fertilization/intracytoplasmic sperm injection-embryo transfer(IVF/ICSI-ET)treatment at the Reproductive Hospital Affiliated to Shandong University from January 1,2017,to December 31,2021,were retrospectively analyzed.According to the transfer protocol,the patients were divided into a fresh embryo transfer group(262 cases)and a freeze-all and frozen embryo transfer group(184 cases).The basic data,controlled ovarian hyperstimulation(COH)data,pregnancy out-comes and perinatal outcomes of the two groups were compared,and appropriate statistical methods were used for data analysis.Results:There were statistically significant differences in body mass index(BMI)basal,follicle-stimulating hormone(FSH)and AMH level between the two groups.However,there were no statistically significant differences in pregnancy and perina-tal outcome indicators such as biochemical pregnancy miscarriage rate and clinical pregnancy rate.Overall,the pregnancy outcomes of the FET group were better.The cumulative pregnancy rate of the FET group was higher than that of the fresh embryo transfer group,but the difference did not reach statically significant.Conclusions:In patients with adenomyosis and DOR,FET may have an advantage in overall pregnancy outcomes,and whole embryo cryopreservation may increase the cumulative pregnancy rate.When making clinical decisions,factors such as age,o-varian reserve function and the number of retrieved eggs should be comprehensively considered to develop personalized treatment plans.关键词
