Abstract
Objective:To investigate the fetal and maternal outcomes and risk factors of adverse pregnancy events(APEs)in pregnancies with undifferentiated connective tissue dis-ease(UCTD).Methods:This retrospective study described the outcomes of 113 pregnancies in patients with UCTD.The patients were divided into APEs group(n=71)and non-APEs group(n=42).The differences in clinical manifestations,laboratory data and treatments between the two groups were compared,and the impact factors of APEs in pregnancies with UCTD were ana-lyzed.Results:Among the 113 pregnancies with UCTD,there were 99(87.6%)live births,in-cluding 19(16.8%)preterm delivery,and 3(2.7%)stillbirths,11(9.7%)miscarriage,3(2.7%)pre-eclampsia,6(5.3%)fetal growth restriction(FGR),7(6.2%)oligohydram-nios,19(16.8%)premature rupture of membranes(PROM),8(7.1%)postpartum hemor-rhage(PPH),and 11(9.7%)small for gestational age infants(SGA).Compared the patients without APEs,the patients with APEs had a higher proportion of assisted reproductive technolo-gy(ART)(22.5%vs 7.1%,P=0.034),higher positive rate of anti-cardiolipin antibodies(aCL)(26.8%vs 9.6%,P=0.028),less use of low-dose glucocorticoid(60.6%vs 81.0%,P=0.025),earlier gestational week of delivery[37.1(33.1,38.1)weeks vs 38.4(38.1,39.1)weeks,P<0.001]and lower birth weight of the newborn[2930.0(2350.0,3235.0)g vs 3300.0(3057.5,3485.0)g,P<0.001].There were no significant differences(P>0.05)between the two groups in the history of adverse pregnancy nor recurrent spontaneous abortion(RSA).Multivariate logistic regression analysis showed that the administration of low-dose glucocorticoids(OR=0.353,95%CI:0.139~0.899,P=0.029)was an independent protective factor for APEs in UCTD.Conclusion:APEs are common in patients with UCTD.ART and positive aCL are risk factors for the development of APEs.Low-dose glucocorticoids is recommended in pregnancies with UCTD for its favorable effects to pregnancy outcomes.关键词
