PICC留置患者静脉内皮细胞损伤及血栓形成的生物学机制探讨
Discussion on biological mechanism of venous endothelial cell injury and thrombosis in patients with PICC indwelling

Abstract
To explore the influence of the indwelling time of peripheral inserted central catheter(PICC)on venous endothelial function and thrombosis,this work selected 126 patients who received PICC catheterization in the intravenous therapy outpatient department of our hospital from January 2024 to January 2025 and had an indwelling period of ≥14 d as the research subjects(divided into short-term group 14~30 d,medium-term group 31~60 d,and long-term group>60 d according to the indwelling period,with 42 cases in each group),and 42 healthy volunteers were selected as the control group.The levels of serum intercellular adhesion molecule-1(ICAM-1),vascular cell adhesion molecule-1(VCAM-1),endothelin-1(ET-1)and nitric oxide(NO)were detected by ELISA,D-dimer was determined by immunoturbidimetry,and fibrinogen content was detected by clauss method.Meanwhile,the venous inner diameter,hemodynamic parameters and thrombotic elastic characteristics were measured by the high-frequency color doppler ultrasound system.The results showed that the levels of serum ICAM-1,VCAM-1 and ET-1 in the long-term indwelling group are significantly higher than those in the control group(P<0.001),the level of NO is significantly decreased(P<0.001),and the ET-1/NO ratio is increased.The levels of D-dimer and fibrinogen are gradually increased with the extension of indwelling time,and are significantly higher in the long-term group than in the control group(P<0.05).The platelet count shows a trend of first increasing and then decreasing.The incidence of thrombosis in the long-term group is significantly higher than that in the control group(28.6%vs 0%,P<0.01),with a gradient decrease in peak flow velocity(PSV).Hyperechoic organized thrombosis is observed in the long-term group.The above results indicated that long-term indwelling of PICC can increase the risk of thrombosis by damaging venous endothelial function and activating the coagulation system.ICAM-1,ET-1/NO and D-dimer can serve as potential markers for early warning of thrombosis.关键词
