DRGs-PPS支付模式对首发结肠癌手术患者住院费用的影响
Impact of DRGs-PPS payment model on hospitalization costs for patients undergoing primary colon cancer surgery
摘要
Abstract
Objective To explore the impact of DRGs-PPS payment model on hospitalization expenses for primary colon cancer surgery patients,in order to provide a basis for improving medical insurance management sys-tem,reducing medical expenses,continuously improving the operational efficiency and effective allocation of the medical security system.Method The medical records of 624 patients who underwent primary colorectal cancer surgery in two grade A hospitals in Nanchang from January 2022 to March 2024 and were paid by DRGs-PPS were analyzed,and a non-conditioned Logistic regression analysis model was constructed to test the influencing factors of hospitalization costs of patients with primary colorectal cancer surgery.Result Univariate analysis showed that there were differences between the two groups in terms of length of hospital stay,number of other diagnoses,ad-mission status,surgical method,DRG grouping,drug costs,testing costs,and payment methods(P＜0.05).The results of the multi-factor Logistic regression analysis showed that GB23 group(OR=3.187),laparoscopic surgery(OR=2.898),other diagnoses of more than 3 items(OR=2.718),hospitalization days of more than 10d(OR=2.361),drug costs(OR=3.435)and test costs(OR=3.353)were the influencing factors of hospi-talization costs in patients with primary colorectal cancer surgery(all P＜0.05).Conclusions In the framework of DRGs-PPS payment mode,the hospitalization costs of colorectal cancer patients were different under different DRG groups.More accurate management strategies should be adopted for different DRG groups and patients to ef-fectively control medical expenses and improve the efficiency of medical insurance management.关键词
