基于ICF理论的脑卒中失能老人居家照护模式的探索与实践
Home-based care mode for the elderly with stroke disability based on the ICF framework

Abstract
Objective To develop a home-based care protocol for the elderly stroke survivors with disabil-ities and to evaluate its implementation after clinical application.Methods The study selected 132 pairs of stroke patients with mild-to-moderate disability,admitted to a medical group between September 2022 and October 2023,and their primary caregivers as research subjects,and then assigned them into a control group and an intervention group.The control group performed conventional inpatient care and discharge follow-up,while the intervention group implemented a home-based care intervention program based on the ICF framework for the stroke-incapacita-ted elderly.Assessments were conducted before the intervention and 4 weeks after the discharge on the quality of life and the family APGAR score of the incapacitated elderly as well as the caregiver burden and the caregiving competence of the primary caregiver.Results After the intervention,there was significantly statistical difference in caregiver burden score between the control group and the intervention group[(74.15±7.21)vs.(61.09±11.14)];in the caregiving ability[(50.24±5.85)vs.(61.00±7.59)],quality of life[(68.66±9.89)vs.(77.87±8.59)],and family APGAR score[(12.97±2.52)vs.(14.01±2.58)](all P＜0.05).Conclu-sions The home-based care mode for stroke survivors with disabilities,which is based on the ICF framework,not only effectively alleviates caregiver burden and enhances caregiving competence but also improves patients'quality of life and family APGAR scores.The intervention protocol provides practical guidance for establishing a compre-hensive health care system for disabled stroke survivors.关键词
