Abstract

Objective To investigate the effects of ulinastatin(UTI)on the ultrastructural changes in the hippocampus,ATP concentration,and glutamate transporter-1(GLT-1)mRNA expression levels in rats after cardiopulmonary resuscitation(CPR).Methods Ninety adult male SD rats were randomly divided into three groups:the sham-operation group(S group),the normal saline resuscitation group(NS group),and the ulinastatin resuscitation group(UTI group),with 30 rats in each group.The S group underwent anesthesia,tracheotomy,and vascular puncture only.The NS and UTI groups underwent conventional resuscita-tion after asphyxiation-induced cardiac arrest.Two minutes and 1.5 hours after restoration of spontaneous circulation(ROSC),the UTI group received ulinastatin(105 U/kg)via femoral vein injection,while the NS group received an equivalent dose of nor-mal saline.In S group,2.5,4,and 6 hours after tracheal intubation and vascular puncture,and in the NS group and the UTI group,2.5,4,and 6 hours after ROSC,10 rats were sacrificed and their brains were harvested.The ultrastructural changes in the hippocampus were observed using transmission electron microscopy.The ATP content in the hippocampus was measured using high-performance liquid chromatography.The expression levels of GLT-1 protein were determined by Western blot,and the chan-ges in GLT-1 mRNA were measured by real-time PCR(RT-PCR).Results Compared with the S group,the ATP content in the hippocampus of NS group rats was significantly reduced at 2.5,4,and 6 hours after ROSC(P<0.01).At 2.5 hours after ROSC,the ATP concentration in the UTI group was significantly higher than that in the NS group(P<0.05),but still lower than that in the S group.At 4 and 6 hours after ROSC,the ATP concentration in the UTI group was significantly higher than that in the NS group(P<0.01)and approached the level of the S group.Transmission electron microscopy showed that at 6 hours af-ter ROSC,the hippocampal neurons in the NS group exhibited obvious ultrastructural damage,while the damage in the UTI group was less severe.The expression of GLT-1 protein and mRNA in the S group remained relatively stable,while the expression of GLT-1 protein and mRNA in the NS group was significantly lower than that in the S group.At 2.5,4,and 6 hours after ROSC,the expression of GLT-1 protein and mRNA in the UTI group was significantly higher than that in the NS group(P<0.05 or P<0.01)and approached the level of the S group.Conclusion UTI may exert its effects by upregulating the transcription of the GLT-1 gene,thereby increasing the expression of GLT-1 protein.This mechanism may,to some extent,alleviate the pathological damage in the hippocampal tissue of rats following CPR and exert a neuroprotective effect in rats after cardiac arrest.
