全髋关节置换术和PFNA治疗不稳定性股骨转子间骨折的临床疗效

Abstract
Objective To explore the clinical effects of total hip replacement(THA)and proximal femoral antirotation intr-amedullary nail(PFNA)in the treatment of unstable femoral intertrochanteric fractures.Methods A total of 62 patients with unstable intertrochanteric femoral fractures were randomly divided into the THA group(28 patients)and the PFNA group(34 pa-tients).The postoperative parameters,Harris score,quality of life score,and the incidence of postoperative complications were compared between the two groups.Results The operation time of the PFNA group was shorter,with less blood loss and less hos-pitalization cost(P＜0.01);the THA group had a shorter hospital stay,earlier postoperative ambulation time(P＜0.01),higher postoperative Harris hip function score and quality of life score(SF-36)(P＜0.05 or＜0.01),and a lower incidence of postoperative complications(P＜0.01).Conclusion THA for unstable intertrochanteric femoral fractures not only outperforms PFNA in hip joint function and quality of life,but also enables earlier ambulation on the bed,reduces hospital stay time,and ac-celerates the recovery of hip joint function in patients.关键词
