包头地区住院糖尿病足感染患者病原菌特征及临床价值分析
Pathogen characteristics and clinical value analysis of inpatient diabetic foot infection patients in Baotou region

Abstract
Objective To study the distribution characteristics of pathogens and drug sensitivity test results in hospitalized di-abetic foot infection(DFI)patients in the Baotou region,and to explore the factors influencing the degree of infection in these patients.Methods A total of 128 patients diagnosed with DFI,admitted to multiple centers(The Second Affiliated Hospital of Baotou Medical College,The First Affiliated Hospital of Baotou Medical College,Baotou City Central Hospital,Sinopharm North Hospital,and Baotou Eighth Hospital)in Baotou from January 2020 to June 2024,were selected as the study participants.The patients were categorized into three groups-mild,moderate,and severe-according to the Wagner grading system.Clinical data,pathogen distribution characteristics,drug sensitivity test results,and factors influencing in-fection severity were analyzed.Results Among the 128 samples,141 strains of pathogens were cultured,including 88 Gram-positive bacteria(62.4%)and 53 Gram-negative bacteria(37.6%),with no fungi cultured.The main Gram-positive bacteria included Staphylococcus aureus(48 strains,54.5%),Enterococcus faecalis(13 strains,14.8%),and Staphylococ-cus epidermidis(7 strains,8.0%).The main Gram-negative bacteria included Escherichia coli(15 strains,28.3%),Kleb-siella pneumoniae(11 strains,20.8%),and Enterobacter cloacae(9 strains,17.0%).A total of 29 multidrug-resistant strains were detected,primarily methicillin-resistant Staphylococcus aureus(MRSA;18 strains,62.1%)and extended-spectrum beta-lactamases(ESBLs)-producing bacteria(10 strains,34.5%).In different infection severity groups,as the severity of infection increased,the proportion of Gram-positive bacterial infections gradually decreased,while the proportion of Gram-negative bacterial infections gradually increased.Gram-positive bacteria were most sensitive to linezolid,tigecy-cline,and vancomycin,while exhibiting higher resistance to clindamycin,erythromycin,and penicillin G.Gram-negative bacteria were most sensitive to amikacin,piperacillin/tazobactam,cefoperazone/sulbactam,and carbapenem,and showed resistance to second-generation cephalosporins.Logistic regression analysis indicated that neutrophil count(NEU),albumin level,and type of infecting pathogen were independent factors influencing the severity of infection in DFI patients(P<0.05).Conclusion In the Baotou region,hospitalized DFI patients are predominantly infected by Gram-positive bacteria,but the infection rate of Gram-negative bacteria is higher in patients with more severe infections.There are differences in the sensitivity and resistance of different pathogens to various antibiotics.NEU,albumin levels,and the type of infecting patho-gen are independent factors influencing the severity of infection in DFI patients.关键词
