三间隙引流术治疗肛周脓肿的临床疗效及对患者肛门功能的影响
Clinical efficacy of three-space drainage surgery in treating perianal abscess and its impact on anorectal function

Abstract
Objective To explore the clinical efficacy of three-space drainage surgery in the treatment of perianal abscess(PA)and its impact on anorectal function,aiming to provide a reference for optimizing clinical treatment strategies for PA.Methods A retrospective analysis was conducted on the clinical data of 202 PA patients treated at Chengdu Integrated TCM &Western Medicine Hospital from January 2021 to January 2024.According to the differences in surgical methods,the pa-tients were assigned to a control group and an observation group,with 101 cases in each group.The control group received conventional incision and drainage;while the observation group,three-space drainage surgery.Clinical efficacy,periopera-tive indicators,pain levels,changes in anorectal function,and the occurrence of complications were compared between the two groups.Results The overall therapeutic response rate was 95.05％(96/101)in the control group and 98.02％(99/101)in the observation group,with no statistically significant difference(x2=0.592,P=0.442).The observation group had significantly less intraoperative blood loss,shorter hospital stays,and earlier time to first defecation compared to the control group(P＜0.05).On postoperative day 1,the visual analog scale(VAS)scores for pain were not significantly dif-ferent between the groups(P＞0.05);however,the VAS scores on postoperative days 7 and 30 were significantly lower in the observation group than in the control group(P＜0.05).On postoperative day 1,the Wexner score for anorectal function in the observation group was significantly lower than that in the control group(P＜0.05);there were no significant differ-ences between the two groups in the Wexner score on postoperative days 7 and 30(P＞0.05).The complication rate in the observation group was significantly lower than that in the control group(3.96％[4/101]vs.11.88％[12/101];x2=4.344,P=0.037).Conclusion The clinical efficacy of three-space drainage surgery for treating PA is comparable to con-ventional incision and drainage,but it has distinct advantages in reducing postoperative pain,protecting anorectal function,and lowering the risk of postoperative complications,justifying its clinical promotion.关键词
