MPV、LYC及二者比值与子痫前期患者剖宫产术后发生深静脉血栓的关系
Relationship of mean platelet volume,lymphocyte count and their ratio to post-cesarean deep vein thrombosis in preeclampsia patients

Abstract
Objective To investigate the relationship between mean platelet volume(MPV),lymphocyte count(LYC)and their ratio,and the occurrence of deep vein thrombosis(DVT)after cesarean delivery in patients with preeclampsia(PE),thereby providing a reference for clinical management.Methods From February 2022 to October 2024,112 patients with PE who underwent cesarean section at Xingtai Central Hospital were enrolled.According to whether postoperative DVT oc-curred,patients were assigned to a DVT group(n=27)and a non-DVT group(n=85).Patients in the DVT group were further stratified by the Wells clinical score into mild(≤ 0 point,n=11),moderate(1-2 points,n=10)and high(≥ 3 points,n=6)subgroups.MPV and LYC were measured using an automated hematology analyzer,and the MPV/LYC ratio(MPVLR)was calculated.Receiver operating characteristic(ROC)curves were used to analyze the predictive value of MPV,LYC and MPVLR for post-cesarean DVT in PE patients.Multivariate logistic regression was adopted to identify fac-tors influencing postoperative DVT in PE patients.Results MPV and MPVLR were higher and LYC was lower in the DVT group than in the non-DVT group,with statistically significant differences(P＜0.05).Comparisons of MPV,LYC and MPVLR across Wells score subgroups showed significant differences in MPV,LYC,and MPVLR(P＜0.05):MPV and MPVLR increased and LYC decreased progressively from the mild to the moderate to the high-risk groups(P＜0.05).MPV and MPVLR were positively correlated with the Wells clinical score,while LYC was negatively correlated with the Wells clinical score(P＜0.05).The areas under the ROC curves(AUC)for MPV,LYC and MPVLR to predict post-cesar-ean DVT were 0.741(95％CI:0.696-0.791),0.863(95％CI:0.818-0.908)and 0.922(95％CI:0.872-0.967),re-spectively.Multivariate analysis showed that BMI ≥28.00 kg/m2 at delivery(OR=1.799;95％CI:1.182-2.736),base-line systolic blood pressure ≥140 mmHg(1 mmHg=0.133 kPa)(OR=1.640;95％CI:1.126-2.390),comorbid with varicose veins(OR=2.121;95％CI:1.302-3.456),MPV ≥10.85 fL(OR=2.545;95％CI:1.650-3.924),LYC ≤1.68×109/L(OR=2.392;95％CI:1.566-3.652),and MPVLR ≥ 6.46(OR=3.377;95％CI:1.947-5.858)were independent risk factors for post-cesarean DVT in these patients(P＜0.05).Conclusion MPV,LYC and their ratio are closely correlated with the occurrence of DVT after cesarean delivery in patients with PE and may serve as important predic-tive indicators,with the MPVLR showing superior predictive performance.关键词
