Relationship between serum soluble α-Klotho protein and nerve injury markers in patients undergoing craniotomy for traumatic brain injury and the predictive value of their combination on survival outcome

Abstract
Objective To investigate the relationship between serum soluble α-Klotho protein and nerve injury markers in patients undergoing craniotomy for traumatic brain injury(TBI)and the predictive value of their combination on survival outcome.Methods Totally 112 patients with TBI who underwent craniotomy from March 2021 to March 2024 were selected and divided into mild to moderate reconstruction according to the degree of illness.At the same time,the survival outcome of the patients after 28 days was observed and divided into survival group(82 cases)and death group(30 cases).The baseline data and serum levels of solubleα-Klotho protein,nerve injury markers(S100β,neuron-specific enolase(NSE))were compared among these groups.Pearson method,Logistic regression method and ROC curve were used to analyze the correlation,influencing factors and predictive value.Results The levels of serum S100β and NSE in the moderate group were lower than those in the severe group,and the levels of serum S100β and NSE in the mild group were higher than those in the severe group(r=-0.285,-0.277,P=0.003,0.004).Pearson correlation results showed that serum soluble α-Klotho protein was negatively correlated with S100 β and NSE(P<0.05).Logistic regression analysis showed that acute physiology and chronic health status assessment Ⅱ(APACHEⅡ)score and serum S100β and NSE were the risk factors that affected the prognosis and death risk of TBI craniotomy patients,while Glasgow coma scale(GCS)score,albumin and serum soluble α-Klotho protein were the protective factors(OR=2.646,2.670,2.656,0.520,0.557,0.531,P=0.003,0.005,0.004,0.006,0.007,0.005).ROC curve analysis showed that the AUC of serum soluble α-Klotho protein combined with S100β and NSE in predicting the survival outcome of patients undergoing TBI craniotomy was 0.922(95%CI:0.874-0.970),with a sensitivity of 90.00%and a specificity of 81.71%.The results of Delong test showed that the AUC of joint detection was higher than the single index(Z=3.618,2.764,2.855,P<0.001,0.006,0.004).Conclusion Serum soluble α-Klotho protein in patients undergoing TBI craniotomy is negatively correlated with S100β,NSE.Soluble α-Klotho protein,S100β,NSE are not only influencing factors of survival outcome of patients undergoing TBI craniotomy,but also have good predictive value.关键词
