ADC与动态增强MRI对原发性中枢神经系统淋巴瘤的诊断价值
Diagnostic value of ADC with dynamic enhanced MRI for primary CNS lymphoma

Abstract
Objective To analyze the clinical value of the apparent diffusion coefficient(ADC)and dynamic enhanced MRI in the diagnosis of primary central nervous system(CNS)lymphoma.Methods From January 2021 to June 2024,92 patients admitted to the Ninth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine were studied,and all enrolled patients underwent dynamic enhanced MRI and diffusion weighted imaging(DWI)with Signa HDx 3.0T MR scanner.Taking postoperative pathological examination results as the"gold standard"for diagnosis,the sensitivity,specificity and accuracy of ADC,dynamic enhanced MRI and the combination for the primary CNS lymphoma.The receiver operating characteristics(ROC)curve of ADC,dynamic enhanced MRI and the combined for the diagnosis of primary CNS lymphoma were made,and the area under the curve(AUC)was measured.Results Postoperative pathology examination results showed that 38 cases of primary CNS lymphoma were diagnosed with a positive rate of 41.30%;31 cases diagnosed primary CNS lymphoma with a positive rate of 32.61%by ADC;30 cases diagnosed primary CNS lymphoma with a positive rate of 33.70%by dynamic enhanced MRI;37 cases diagnosed primary CNS lymphoma with a positive rate of 40.22%by ADC combined with dynamic enhanced MRI.Among the three diagnostic methods,the combination of ADC and dynamic enhanced MRI had the highest sensitivity(97.37%),specificity(96.30%),and accuracy(96.74%)for the diagnosis of primary CNS lymphoma,with statistically significant differences(P<0.05).The AUC values of ADC,dynamic enhanced MRI and ADC combined with dynamic enhanced MRI for diagnostic primary CNS lymphoma were 0.815 9,0.810 4 and 0.913 7,respectively.Conclusion The combined diagnosis of ADC and dynamic enhanced MRI has a good diagnostic efficacy for primary CNS lymphoma,and can effectively improve the sensitivity,specificity and accuracy,with significant clinical application value.关键词
