Value of SWI asymmetric MHVs combined with DWI in evaluating neurological impairment and cerebral perfusion in patients with cerebral infarction

Abstract
Objective To investigate the value of SWI asymmetric multiple hypointense vessels sign(MHVs)combined with diffusion-weighted imaging(DWI)in evaluating neurological impairment and cerebral perfusion in patients with cerebral infarction.Methods A total of 96 patients with cerebral infarction who received DWI and SWI examination were enrolled in this study.They were divided into two groups according to the situation of neurological impairment.Clinical data of the two groups were compared to analyze the differences in infarct lesion volume and the grade of MHVs between patients with different neurological impairments.The receiver operating characteristic(ROC)curves were used to analyze the value of above differential indicators in evaluating neurological impairments.The differences in cerebral perfusion parameters of patients with different grades of MH Vs were analyzed.Results There were statistically significant differences in the infarct lesion volume scores and SWI asymmetric MHVs between the two groups(P<0.05).ROC curves indicated that infarct lesion volume score was similar to SWI asymmetric MHVs score for evaluating neurological impairment in stroke(Z=0.760,P=0.447).A joint evaluation model was constructed based on Logistic regression analysis.The value of joint evaluation was significantly higher than that of infarct lesion volume score or SWI asymmetric MHVs score alone(Z=2.476,3.246,P=0.013,0.001).Multivariate Logistic regression analysis showed that both infarct lesion volume score and SWI asymmetric MHVs score were influencing factors for neurological impairment in stroke patients(P<0.05).The reconstructed images of SWI showed 28 cases of grade Ⅰ,33 cases of grade Ⅱ,and 35 cases of grade Ⅲ.There were statistically significant differences in cerebral perfusion parameters among patients with different grades of MHVs.Cerebral blood flow and volume increased with the increase of the grade of MHVs(P<0.05).Spearman correlation analysis found that the grade of MHVs was positively correlated with perfusion parameters(r=0.653,P<0.05).Conclusion SWI asymmetric MHVs combined with DWI can effectively help to evaluate neurological impairment and cerebral perfusion in patients with cerebral infarction.关键词
