基于"脾胃转枢"论治肾病综合征
On Treatment of Nephrotic Syndrome Based on the Theory of"Spleen-Stomach as the the Pivotal Hub"

Abstract
The pathogenesis of nephrotic syndrome(NS)involves dysfunction of qi movement and the pivotal mechanism,deficiency of healthy qi,and accumulation of turbidity.This shares a close structural relationship with the impairment of the function of"spleen-stom-ach as the the pivotal hub".Spleen-stomach weakness and qi-blood deficiency form the root of the disease,while internal retention of dampness is the manifestation of disease progression.Dampness-heat accumulation is the cause of the persistent nature of the condition,and qi stagnation and blood stasis represent the transformation of chronic illness entering the collaterals.Therefore,clinical treatment should focus on regulating qi movement to restore normal ascending and descending,facilitating the pivotal mechanism to unblock the triple energizer,and reinforcing deficiency while draining turbidity to balance excess and deficiency.For cases with spleen-stomach weakness and insufficient qi-blood production,Modified Center-Supplementing and Qi-Boosting Decoction combined with Land and Wa-ter Two Immortals Elixir is commonly used.This approach aims to tonify qi,lift the clear,consolidate essence,and harmonize qi and blood,thereby reestablishing the stable physiological state of"vigorous spleen-sufficient qi-consolidated essence-controlled water."For cases with dysfunction of spleen-stomach ascending and descending and internal retention of dampness,Modified Stephania Root and Astragalus Decoction combined with Five Substances Powder with Poria is adopted to tonify qi,invigorate the spleen,promote yang to transform qi,and drain dampness to promote urination in order to restore the physiological state of"normal spleen transportation-trans-formed dampness-cleared kidney collaterals."For cases with spleen-stomach disharmony and internal accumulation of dampness-heat,Wonderfully Effective Four Powder and Three Kernels Decoction are used as basic formulas for modification.By pungent-dispersing and bitter-descending,invigorating the spleen to resolve dampness,and detoxifying to discharge turbidity,this approach reestablishes the physiological state of"restored spleen transportation-cleared dampness and heat-pacified kidney collaterals."For cases with spleen-stomach qi stagnation and internal obstruction of blood stasis,Modified Blood Mansion Stasis-Expelling Decoction combined with Costus-root and Amomum Six Gentlemen Decoction can be used for modification.By regulating qi movement,activating blood to unblock collat-erals,and reducing masses to dissipate binds,this approach reverses the vicious cycle of"qi stagnation-blood stasis-water retention-toxin damage."Thus,the clear yang ascends while the turbid yin descends,essence is consolidated and water-dampness is naturally regula-ted.In this way,severe illness can be cured and chronic conditions can be alleviated.关键词
