Syndrome Differentiation and Treatment of Diabetes Mellitus Type 2 Retinopathy Based on"Spleen-Stomach Deficiency Leading to Obstruction of the Nine Orifices"

Abstract
The nine orifices of the human body are divided into the upper yang orifices and the lower yin orifices.Their normal function depends on the nourishment of qi,blood,and body fluids.The spleen and stomach,located in the middle energizer,serve as the pathway for the ascending and descending of qi between the upper yang orifices and the lower yin orifices.Either excess or deficiency of spleen-stomach function can affect the functioning of the nine orifices to some extent.The occurrence and development of diabetic retinopathy are closely related to spleen-stomach deficiency.The core pathogenesis is the failure of the spleen to ascend clear yang,and the obstruc-tion of the eye orifices is a critical pathological link.Earth deficiency and water depletion persist throughout the entire course of diabetic retinopathy.Treatment should focus on reinforcing the earth to elevate yang and control fire,reinforcing the earth to boost qi and regulate the middle energizer,and elevating yang to purge fire and harmonize yin and yang.Commonly used drugs for reinforcing the middle ener-gizer and boosting qi include Huangqi(Radix Scutellariae)and Renshen(Radix Ginseng)to promote the ascending function,as well as Baizhu(Rhizoma Atractylodis Macrocephalae),Gancao(Radix Glycyrrhizae),and Chenpi(Pericarpium Citri Reticulatae),etc.to pro-tect the spleen and stomach and preserve stomach yang.At the same time,drugs such as Chaihu(Radix Bupleuri)and Xingren(Semen Armeniacae Amarum)are added to regulate the qi movement of the liver and lung,thereby adjusting the overall circulation of qi through-out the body.For elevating yang and purging fire,Decoction for Tonifying the Spleen and Stomach and Draining Yin Fire and Elevating Yangcan be used to elevate floating yang qi and drain stagnant yin fire.关键词
