Dang Zhongqin's Experience in Treating Chronic Atrophic Gastritis

Abstract
Professor Dang Zhongqin believes that chronic atrophic gastritis(CAG)is characterized by syndrome of deficient root cause and excessive manifestation,with a mixture of deficiency and excess.Spleen-stomach deficiency,transportation dysfunction of the middle energizer,spleen-stomach disturbance in ascending and descending are the root causes,while qi stagnation,dampness accumulation,pathogenic toxins,and blood stasis are the secondary manifestations.The disease is located in the stomach and is related to the liver and spleen.The basic pathogenesis of CAG is transportation dysfunction of the middle energizer,and spleen-stomach disturbance in ascend-ing and descending.Professor Dang proposes that the treatment should focus on invigorating the spleen,regulating qi,promoting blood circulation,removing blood stasis,detoxifying,and eliminating dampness.He has developed an empirical formula,Mass-Eliminating and Stomach-Harmonizing Decoction,for modification.Baihuasheshecao(Herba Hedyotis Diffusae)-Banzhilian(Herba Scutellariae Barba-tae)is commonly used in Professor Dang's clinical practice for treating CAG,serving to clear heat,detoxify,expel wind,and remove dampness.For liver-stomach qi stagnation syndrome,Zhishi(Fructus Aurantii Immaturus)and Chenpi(Pericarpium Citri Reticulatae),etc.are added to soothe the liver,descend qi,invigorate the spleen,and harmonize the middle energizer.For spleen deficiency with ex-cessive dampness syndrome,Huoxiang(Herba PogoStemonis)and Peilan(Herba Eupatorii)are added to invigorate the spleen and re-solve dampness.For dampness-heat syndrome,Changpu(Rhizoma Acori Tatarinowii)and Cangzhu(Rhizoma Atractylodis)are added to clear heat,detoxify,drain dampness,and reduce turbidity.For stomach yin deficiency syndrome,Shihu(Caulis Dendrobii)and Shengdi-huang(Radix Rehmanniae)are added to astringe yin and promote fluid production.For spleen-stomach deficiency-cold syndrome,Fuzi(Radix Aconiti Lateralis Preparata)and Ganjiang(Rhizoma Zingiberis)are added to warm the stomach and disperse cold.For blood stasis obstructing collaterals syndrome,Jiuxiangchong(Pheretima)and Wulingzhi(Faeces Trogopterori)are added to activate blood circulation,remove stasis,and alleviate pain.Professor Dang emphasizes that clinical medication should be based on"disease differenti-ation and syndrome differentiation as the foundation,with symptom-based modifications as the key."Under the premise of disease and syndrome differentiation,flexible drug combinations should be tailored to address accompanying and deteriorated symptoms beyond the main symptoms.For poor appetite,add fried Maiya(Fructus Hordei Germinatus),charred Shanzha(Fructus Crataegi),and charred Shenqu(Massa Fermentata Medicinalis).For epigastric and abdominal distension,add Binglang(Semen Arecae)and fried Laifuzi(Se-men Raphani).For hiccups and regurgitation,add Sharen(Fructus Amomi)and Daizheshi(Haematitum).For frequent stomach pain,add Wulingzhi(Faeces Trogopterori)and Chuanlianzi(Fructus Toosendan).For loose stools,add fried Baizhu(Rhizoma Atractylodis Macrocephalae)and Yiyiren(Semen Coicis).For obvious constipation,add wine-processed Dahuang(Radix et Rhizoma Rhei)and Yuliren(Semen Pruni).Additionally,Professor Dang stresses the integration of disease and syndrome,mutual reference of Chinese and Western medicine,simultaneous treatment of body and mind,and focusing on the patient.For severe emotional stagnation accompanied by anxiety,irritability,and insomnia,add Chaihu(Radix Bupleuri),Suanzaoren(Semen Ziziphi Spinosae),raw Longgu(Os Draconis),etc.to calm the mind,stabilize the will,soothe the liver,and relieve depression on the basis of the original formula.关键词
