加味清胰化积方外敷联合热奄包治疗晚期胰腺癌腹水临床研究
Clinical Observation on the Treatment of Advanced Pancreatic Cancer with Ascites by External Application of Modified Qingyi Huaji Formula Combined with Hot Compress

Abstract
Objective To evaluate the efficacy and interaction of external application of modified Qingyi Huaji Formula combined with hot compress in treating advanced pancreatic cancer with ascites.Methods A total of 48 patients with advanced pancreatic cancer and ascites treated in the Department of Integrated Traditional Chinese and Western Medicine,Minhang Hospital of Fudan University Shanghai Cancer Center from May 2021 to May 2024 were included.According to the factorial design,the patients were allocated via the random number table method into four groups:diuretic(A),diuretic+hot compress(B),diuretic+external application(C),and diuretic+external application+hot compress(D),with 12 patients in each group.After two weeks of treatment,the maximum fluid level diameter of ascites,abdominal circumference,24-h urine volume,TCM symptom score,and adverse reactions were compared before and after treatment.Results The total response rates in groups C and D were higher than those in groups A and B(P＜0.05).After treatment,the decreases in maximum fluid level diameter of ascites in groups C and D were larger than those in groups A and B(P＜0.05).The decrease in abdominal circumference in group D was greater than those in groups C,B,and A(P＜0.05),and the decrease in group C was greater than those in groups A and B(P＜0.05).The increase in 24-h urine volume followed the order of group D＞group C＞group B＞group A(P＜0.05).After treatment,the changes in abdominal distension pain,flatus,and fatigue scores in groups C and D were greater than those in groups A and B(P＜0.05).The interaction effect between modified Qingyi Huaji Formula and hot compress on the 24-h urine volume was significant(P＜0.05).Conclusion The external application of modified Qingyi Huaji Formula with or without hot compress can effectively reduce the amount of ascites and alleviate clinical symptoms in treating advanced pancreatic cancer with ascites,demonstrating certain short-term efficacy and good safety.The combined treatment can synergistically increase the urine volume of the patients.关键词
