司美格鲁肽联合达格列净对早期2型糖尿病肾病的疗效及内皮功能的影响
Effect of Semaglutidecombined with Dapagliffozin on early type 2 diabetic nephropathy and endothelial function

Abstract
Objective To explore the effect of Dapagliflozin combined with Semaglutide on patients with early type 2 diabetic nephropathy(T2DN)and its impact on endothelial function.Methods A total of 80 patients with early T2DN combined with microalbuminuria who were admitted from January to October 2023 were collected and divided into the observation group(n=40)and the control group(n=40)by random number table method.During the research period,there were 2 patients in each group who voluntarily withdrew from the study without timely follow-up visits.The observation group was treated with Dapagliflozin combined with Semaglutide,while the control group was treated with Dapagliflozin combined with Insulin Glargine.The glycated hemoglobin(HbA1c),body mass index(BMI),blood glucose,blood lipid,cystatin C(CysC),urine albumin-to-creatinine ratio(UACR),nitric oxide,and endothelin-1 before and after treatment were compared between the two groups.Meanwhile,the occurrence of adverse events was recorded.Results After treatment,fasting blood glucose,2-hour postprandial blood glucose,HbA1c,BMI,low-density lipoprotein cholesterol and cholesterol in both groups decreased compared with those before treatment.Moreover,the levels of HbA1c and BMI in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of CysC and UACR in the observation group decreased compared with those before treatment and in the control group(P<0.05).After treatment,the nitric oxide in the observation group increased compared with that before treatment,and endothelin-1 decreased compared with that before treatment.Moreover,the nitric oxide level in the observation group was higher than that in the control group(P<0.05).No serious adverse reactions occurred in either group.Conclusion Dapagliflozin combined with Semaglutide in the treatment of early T2DN can improve the glycolipid metabolism of patients,reduce proteinuria,improve endothelial function,delay the progression of DN,and has good safety.关键词
