无创正压联合早期俯卧位通气治疗急性呼吸衰竭的效果
Clinical effect of non-invasive positive pressure ventilation combined with early prone position ventilation in the treatment of patients with acute respiratory failure

Abstract
Objective To explore the clinical effect of non-invasive positive pressure ventilation(NPPV)combined with early prone position ventilation(PPV)in the treatment of acute respiratory failure(ARF).Methods A total of 80 patients with ARF admitted from February 2021 to February 2024 were selected as the research subjects and divided into group A and group B by random number table method,with 40 patients in each group.Both groups were given conventional treatment.Group A received NPPV in the prone position on the basis of conventional treatment,while group B received NPPV in the supine position on the basis of conventional treatment.The blood gas analysis indicators[arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2),arterial oxygen saturation(SaO2)],physiological indicators[heart rate,respiratory rate,oxygenation index],and psychological states[Self-Rating Anxiety Scale(SAS)score,Self-rating Depression Scale(SDS)score]at admission and 24 h after admission as well as evaluation indicators of therapeutic effect(rate of endotracheal intubation within 24 h,incidence of complications and satisfaction with hospitalization)were compared between the two groups.Results At 24 h after admission,PaO2,SaO2 and oxygenation index in group A were all higher than those in group B,while PaCO2,heart rate and respiratory rate were all lower than those in group B(P<0.05,P<0.01).The hospitalization satisfaction of group A was higher than that of group B(P<0.05),while the SAS score,SDS score,rate of endotracheal intubation within 24 h and incidence of complications in group A were lower than those in group B(P<0.05).Conclusion The implementation of NPPV combined with early PPV for patients with ARF can not only improve the physiological indicators of patients,but also regulate their psychological state and enhance their satisfaction during hospitalization.关键词
