Chain-mediated effects of fear of cancer recurrence on parenting anxiety in young and middle-aged breast cancer patients

Abstract
Objective To explore the relationship between fear of cancer recurrence(FCR)and parenting worries among middle-aged and young breast cancer patients,and to analyze the impact of co-parenting and family cancer avoidance communication on parenting concerns.Methods From January to December 2024,with convenience sampling,261 middle-aged and young breast cancer patients hospitalized in breast surgery department of two tertiary grade-A hospitals in Zhengzhou were selected as research subjects,and they were surveyed using General Information Questionnaire,Parenting Concerns Questionnaire,and Cancer Recurrence Worry Scale,Family Avoidance Cancer Communication Scale and the Simple Co-parenting Scale.A structural equation model was established to test chain mediation effects.Results The mediating effect test revealed that FCR had a direct effect on parenting concerns(β=0.434,P<0.001)and influenced parenting concerns through three mediating pathways:separate mediating effect of co-parenting(β=0.108,95%CI:0.058～0.164);separate mediating effect of family cancer avoidance communication(β=0.043,95%CI:0.007～0.088);and chain mediation effect of co-parenting-family cancer avoidance communication(β=0.038,95%CI:0.011～0.079).Conclusion Young and middle-aged breast cancer patients experience parenting concerns at a moderately high level,with FCR directly and positively influ-encing their parenting concerns.Co-parenting and family cancer avoidance communication play a mediating role between FCR and par-enting concerns.Healthcare professionals should conduct early and precise assessments of FCR,improve family cancer avoidance com-munication,and enhance family's positive co-parenting capacity to alleviate patients'parenting concerns.关键词
