Barriers to applying best evidence for preventing inpatient child falls from the perspective of caregivers
摘要
Abstract
Objective To investigate the barriers to implementing best evidence for preventing inpatient child falls from the perspective of caregivers,and to inform targeted interventions for this group of population.Methods Guided by the Theoretical Domains Framework(TDF),semi-structured interviews were conducted with 16 caregivers and 22 healthcare providers in a tertiary grade-A children's hospital.Data were analyzed using Colaizzi's seven-step method and thematically categorized into TDF domains.Results Barriers were mapped to five TDF domains,including(1)Knowledge:gaps in assessment tools and cognitive discrepancies between caregivers and healthcare providers;(2)Environmental context and resources:insufficient health education aids and inade-quate intelligence in fall risk notification systems;(3)Motivations and goals:workload concerns in healthcare providers and"treat-ment-first"priority in caregivers;(4)Behavioral regulation:behavioral-institutional misalignment;(5)Beliefs about consequences:insufficient confidence in evidence-based outcomes.Conclusion A multifaceted program integrating intelligent monitoring systems,caregiver-inclusive training programs and staff incentive mechanisms is critical to overcome the barriers and advance evidence-to-practice translation in pediatric fall prevention.关键词
