Abstract
Objective:To explore the mid-and long-term efficacy of arthroscopic lesion clearance surgery against diffuse pigmented villonodulars ynovitis(DPVNS)of the knee.Methods:A single center cohort study design with self-control was employed.Thirty-five patients with DPVNS treated by arthroscopic lesion clearance in the Department of orthopedics in Si-chuan Provincial Bayi Rehabilitation Center from January 2018 to May 2022 were enrolled as the research objects.All pa-tients were followed for more than 3 years,until May 2025.The changes in pain visual analogues cale(VAS),Lysholm knee joint score scale(LKSS),International Knee Documentation Committee(IKDC)score,and range of motion(ROM)before surgery and at the last follow-up were compared and analyzed.Recurrence after surgery through knee joint MRI examination,time of recurrence and the occurrence of complications were recorded.The recurrence was determined by the MRI evaluation results the time of recurrence and the occurrence of complications were recorded.Results:All patients had stage Ⅰ clinical healing of their postoperative wounds,and there were no cases of postoperative infection.Postoperative follow-up was conducted for 36～88 months,with a median follow-up time of 58 months.The VAS score at the last follow-up after surgery was 1.34±0.91,which was significantly lower than preoperative score(P＜0.05).At the last follow-up after surgery,the LKSS score(87.14±6.13),IKDC score(89.89±4.9),and ROM(129.17±7.36)were significantly improved compared to preoperative levels(all P＜0.05).At the last follow-up after surgery,the excellent and good rate of LKSS score was signifi-cantly higher than preoperative score(62.86％vs 0.00％,P＜0.05).Among the 35 patients received treatment,5 patients experienced recurrence,with a recurrence rate of 14.29％.Conclusion:Arthroscopic lesion clearance surgery can achieve ideal mid-to long-term therapeutic effects in knee joint DPVNS,significantly improve patients' symptoms and joint function,but there is still a certain postoperative recurrence rate.关键词
