Exploring the correlation between the Nrf2/HO-1 signaling axis and lumbar spinal stenosis based on the traditional Chinese medicine theory of"Deficiency,Phlegm,and Stasis"

Abstract
Lumbar spinal stenosis(LSS)is a common degenerative spinal disorder.In traditional Chinese medicine(TCM),its pathogenesis is closely associated with the theory of"deficiency,phlegm,and blood stasis".However,the modern biological mechanisms underlying this theory remain underexplored.From the perspective of Western medicine,LSS is primarily related to inflammation and oxidative stress in the surrounding spinal structures.Recent studies have shown that activation of the nuclear factor erythroid 2-related factor 2/heme oxygenase-1(Nrf2/HO-1)signaling pathway can mitigate LSS by inhibiting the nuclear factor kappa B(NF-κB)signaling pathway.This review aims to explore the therapeutic strategies of TCM based on the"deficiency,phlegm,and stasis"theory from an integrative medicine perspective.By analyzing classical oral herbal formulas commonly used under this theoretical framework,the review seeks biological evidence supporting their ability to upregulate the Nrf2/HO-1 pathway.The findings are intended to validate the role of this theory in regulating molecular signaling pathways involved in LSS.Current evidence indicates that treatment principles such as"tonifying the liver and kidney,strengthening the spleen to resolve phlegm,and activating blood circulation to remove stasis"are clinically effective.Moreover,related studies confirm that these formulas can enhance the expression of the Nrf2/HO-1 pathway,providing a scientific foundation for modernizing TCM theories and improving clinical outcomes in LSS treatment.关键词
