从"正虚络痹积损"辨治特发性肺纤维化咳嗽
Differentiating and Treating Cough in Idiopathic Pulmonary Fibrosis from Perspective of"Deficiency of Healthy Qi,Impediment of Collaterals and Accumulated Damage"

Abstract
Idiopathic pulmonary fibrosis(IPF)is mainly accompanied by irritating dry cough,which seriously affects the quality of life of patients.Professor LI Jiansheng proposed the theory of"deficiency of healthy qi,impediment of collaterals and accumulated damage".It was believed that the main pathogenesis of IPF includes deficiency of lung and kidney,impediment of lung collaterals,and mutual accumulation of phlegm and stasis.Based on this theory,it is expounded that the pathogenesis of cough in IPF is as follows:in the initial stage,deficiency of lung qi affects spleen,resulting in deficiency of lung and spleen.Dysfunction of zang-fu organs leads to the internal generation and accumulation of phlegm and stasis.Ultimately,deficiency of both yin and yang and co-existence of flaccidity and impediment occur,and the pathogenesis varies in different courses of the disease.In the treatment,methods such as tonifying lung qi and strengthening spleen to nourish lung are often adopted,using formulas such as Bufei Decoction(补肺汤),and emphasizing the use of Huangqi(Astragali Radix).Drugs for activating blood and resolving phlegm,dredging collaterals and removing impediment,and insect drugs should be used on the basis of tonification.Drugs for tonifying both yin and yang,and treating accumulated damage comprehensively should be used to treat deficiency of kidney yin and yang.关键词
