82例小肠胃肠间质瘤临床病理特征与治疗决策分析
Analysis of clinicopathological characteristics and treatment decision of 82 cases of small intestinal gastrointestinal stromal tumors

Abstract
Objective To explore the clinicopathological characteristics and the diagnosis and treatment strategies of small intestinal gastrointestinal stromal tumors(SI-GISTs),and provide reference for optimizing the overall management of SI-GISTs.Methods A total of 82 patients with primary SI-GISTs who underwent general surgical treatment in the department of general surgery at Chinese People's Lib-eration Army Air Force Medical Center from March 2019 to March 2025 were included in the study.Clinical data including epidemiology,clinical manifestations,imaging and endoscopic diagnostic efficacy,and surgical methods were collected for retrospective analysis.Results SI-GISTs predominantly affected males(75.6%),with 46.3%of the patients over 60 years old.Gastrointestinal bleeding occurred in 91.5%of the patients.Tumors primarily arose in the jejunum(61.0%),with 69.5%measuring≤5 cm,and 58.5%classified as low-risk.Immuno-histochemistry revealed 96.3%positivity for both CD117 and discovered on GIST-1(DOG-1).Enteroscopy showed the highest detection rate of 81.8%,and the detection rate of abdominal CT was 66.7%.Intraoperative findings included tumor rupture/bleeding(29.3%)and lymph node involvement(17.1%),with a postoperative complication rate of 6.1%.Tumor size strongly correlated with recurrence risk(r=0.81,P<0.01).Conclusions SI-GISTs predominantly occur in elderly males,with gastrointestinal bleeding as the primary manifestation.The high jejunal prevalence and multifocality necessitate comprehensive intraoperative exploration.Enteroscopy is pivotal for diagnosis,while a tiered surgical approach based on tumor size and invasiveness balances the requirments of minimal invasion and radical resection.关键词
