多中心血管内大B细胞淋巴瘤12例临床分析
A multicenter research of clinical analysis of 12 cases with intravascular large B-cell lymphoma

Abstract
Objective To analyze the clinical manifestations,pathological features,treatment and prognosis of intravascular large B-cell lymphoma(IVLBCL).Methods A retrospective analysis was conducted on the clinical data of 12 patients with IVLBCL admitted to the First Affiliated Hospital of Henan Medical University(n=7),the First Affiliated Hospital of Zhejiang University School of Medicine(n=4),and Puyang People's Hospital(n=1)from March 2017 to March 2024.Results Among the 12 patients,there were eight males and four fe-males with an average age of 64 years.The most common symptom was fever(12/12),followed by hearing loss(5/12),chest tightness(4/12),shortness of breath(4/12),abdominal distension(4/12),and poor appetite(4/12).All patients exhibited extranodal involvement,primarily in the lung(8/12),bone marrow(6/12),central nervous system(CNS,5/12),skin(4/12),bone(3/12),liver(2/12),soft tissue(2/12),and kidney(1/12).As for pathological features,the dilated capillary lumen was filled with abnormal lymphocytes with large volume,rich cytoplasm,round nuclei,and inconsistent nucleoli under the light microscope.Immunohistochemistry showed the expressions of cluster of differen-tiation 20(CD20,12/12),CD79a(3/3),Pax-5(4/4),and B-cell lymphoma 2(Bcl-2,8/8),the partial expression of CD5(6/10),and no ex-pression of CD3(0/9)and Epstein-Barr virus-encoded small RNA(EBER,0/7).Among the nine patients who received chemotherapy,eight cases received R-CHOP-like regimen and seven achieved long-term disease-free survival.Among the three patients without chemothera-py,two died early,while one had long-term survival after surgical resection.With an average follow-up of 12.00 months,the median overall survival(OS)was not reached,and the one-year OS rate was 66.7%(8/12).Conclusions The most common symptom of IVLBCL was fever and the most common involving organ was lung.R-CHOP is the primary regimen for IVLBCL,while optimal strategies for CNS involvement and relapse require further investigation.关键词
