单侧双通道内镜手术与经皮内窥镜下腰椎间盘摘除术对老年腰椎间盘突出症患者腰椎功能及复发率的影响
Effects of Unilateral Biportal Endoscopy and Percutaneous Endoscopic Interlaminar Lumbar Discectomy on Lumbar Function and Recurrence in Elderly Patients with Lumbar Disc Herniation

Abstract
Objective To compare the clinical efficacy of unilateral biportal endoscopy(UBE)and percutaneous endoscopic lumbar discectomy via an interlaminar approach(PELD-ILA)in elderly patients with lumbar disc herniation(LDH),and to evaluate their effects on lumbar function and recurrence.Methods A total of 104 elderly patients with LDH treated at our hospital from January 2024 to December 2024 were randomly assigned to Group A(n=52,UBE)or Group B(n=52,PELD-ILA).Surgical efficacy,perioperative indicators,pre-and postoperative Oswestry Disability Index(ODI)scores,inflammatory markers[C-reactive protein(CRP)and interleukin-6(IL-6)],incidence of complications,and recurrence rates were compared between the two groups.Results The excellent-good rate of surgery did not differ significantly between the groups(P＞0.05).Fluoroscopy times were comparable(P＞0.05).Group A had greater intraoperative blood loss and longer operative time and hospital stay than Group B(all P＜0.05).At 3 months postoperatively,ODI scores decreased in both groups versus baseline and were lower in Group A than in Group B(P＜0.05).On postoperative day 1,serum CRP and IL-6 levels increased from baseline in both groups and were higher in Group A than in Group B(P＜0.05).The incidences of complications and the recurrence rates showed no significant between-group differences(both P＞0.05).Conclusion UBE and PELD-ILA yield comparable surgical efficacy and safety in elderly patients with LDH,with both approaches associated with low postoperative recurrence.UBE may provide greater improvement in lumbar function,whereas PELD-ILA shortens operative and hospital time,reduces intraoperative blood loss,and attenuates the inflammatory stress response.关键词
