某癌症区域医疗中心胃癌DRG分组住院费用结构及关联性研究
Study on the Structure and Correlation of Hospitalization Costs for Gastric Cancer DRG Grouping in A Regional Medical Center for Cancer Treatment

Abstract
Objective To study the structure of inpatient expenses and associated factors for gastric cancer cases in a regional medical center specializing in cancer,and provide references for medical institutions to precisely control inpatient expenses,allevi-ate patients'economic burden,and improve the medical insurance payment method.Methods Inpatient data including the first pa-ges of medical records and medical insurance settlement lists of gastric cancer cases from 2022 to 2024 in a regional medical center specializing in cancer were collected and grouped.The coefficient of variation(CV)and the coefficient of reduction in total vari-ance(RIV)were used to evaluate the grouping effect of DRG for gastric cancer cases.The structural change degree analysis meth-od and the new grey relational analysis method were employed to study the changes in expenses and the correlations among internal factors.Results A total of 7,451 cases of gastric cancer-related DRG groups were included in the study,which were classified into 4 ADRG groups and 11 DRG groups.The CV values of all groups except RN11,RN15,and GR13 were less than 1,and the RIV values of the 4 ADRG groups were all greater than 0.7.The GB21 group had the highest average inpatient expense(97,128.93 yuan),while the RN15 group had the lowest(7,255.17 yuan).In terms of structural changes,the overall inpatient expenses of each DRG group in this center showed a downward trend.Among which,the expenses for drugs and consumables mainly de-creased,while those for diagnosis and comprehensive services mainly increased.The correlation degree of drug expenses,diagno-sis expenses,and consumable expenses with total inpatient expenses was higher than that of comprehensive service fees,surgical anesthesia fees,and other fees.Conclusion The center has a relatively high degree of routing in the treatment methods for gastric cancer.By strengthening the control of drug and consumable expenses and optimizing the cost structure,the economic burden on gastric cancer patients can be further reduced,and the rational use of medical insurance funds can be ensured.关键词
