经肌间沟弧形切口引流术对高位肛周脓肿的临床疗效及对肛肠动力学的影响
Clinical efficacy of arc incision drainage through intermuscular groove in patients with high perianal abscess and its influence on anorectal dynamics

Abstract
Objective To explore the clinical efficacy of arc incision drainage through intermuscular groove in patients with high perianal abscess(HPA)and its impacts on anorectal dynamics.Methods A retrospective analysis was conducted on the data of 200 HPA patients admitted to Zhangjiakou Fifth Hospital from January 2021 to December 2023.They were grouped into group A(n=101,performing radial incision drainage)and group B(n=99,performing arc incision drainage through intermuscular groove)according to the different surgical procedures.The efficacy,postoperative pain level,incidence of postoperative complications,postoperative recovery,and anorectal dynamic indicators were compared between the two groups.Results The total effective rate of group B was 95.96％,which was higher than 87.13％ of group A(P＜0.05).The pain numerical rating scale(NRS)of both groups gradually decreased after operation,and there was an in-teraction effect between grouping and time on the changes of NRS(P＜0.05).NRS of group B was lower than that of group A on the 1st,3rd,7th,and 10th day after surgery(P＜0.05).In group B,the incidence of post-operative complications was 5.05％,which was lower than 13.86％of group A(P＜0.05),and appearance time of new epithelium or wound healing was shorter than group A(P＜0.05).3 months after surgery,both groups had lower anal resting pressure(ARP)and anal maximal contraction pressure(AMCP),and Wexner score was higher than before(P＜0.05).And comparing with group A,group B had higher ARP,higher AMCP,and lower Wexner score(P＜0.05).Conclusion Compared with radial incision drainage,arc incision drainage through intermuscular groove shows better efficacy in the treatment of HPA,which is more conducive to reducing postoperative pain,promoting wound healing,and alleviating the impact on anorectal dynamics and fecal continence ability.关键词
