重构"上火"的内涵与外延
Reconstructing the connotation and extension of'shanghuo'

Abstract
'Shanghuo'represents one of the most prevalent clinical presentations in contemporary populations.Solely employing heat-clearing and fire-draining therapies often prove clinically inadequate.Etiological analysis reveals improper medication and dietary practices as a principal predisposing factor,while the pathogenesis fundamentally involves qi stagnation,dysfunction of the zang-fu organs and sanjiao,and perturbation of the seven emotions and five mental dispositions.Consequently,therapeutic intervention necessitates pattern differentiation based on distinct etiological and pathogenic mechanisms,prioritizing primary versus secondary factors while applying flexible treatment strategies.Contemporary research demonstrates correlations between'shanghuo'and modern biological mechanisms including inflammatory responses,immune function,energy metabolism,and gut microbiota.Redefining both the theoretical framework within traditional Chinese medicine and the modern scientific extensions of'shanghuo'will facilitate the development of standardized diagnostic and therapeutic protocols to better guide clinical practice,enable investigation into the biological mechanisms underlying fire-heat onset and progression,potentially identify key pathogenic pathways relevant to major refractory diseases such as tumors and coronary heart diseases,provide transferable research paradigms for preventing or interrupting disease progression.关键词
