脊柱与髋部脆性骨折患者骨代谢生化指标的特征分析及比较
Characterization and comparison of biochemical markers of bone metabolism in patients with fragility fractures in the spine and hip

Abstract
Objective To analyze the characteristics of biochemical markers of bone metabolism(BMBMs)in patients with fragility fractures at the spine and hip and to compare the differences between them.Methods This study included 17,886 patients with spine fragility fracture and 12,572 patients with hip fragility fracture who were hospitalized in Xi'an Honghui Hospital from December 2019 to August 2023,and compared blood calcium/phosphorus levels and the serum levels of alkaline phosphatase(ALP),25 hydroxyvitamin D[25(OH)D],parathyroid hormone(PTH),typeⅠprocollagen N telopeptide(P1NP)and typeⅠcollagen C-terminal peptide(CTX)differences.Results The proportion of patients with low calcium,low phosphorus,high PTH,high P1NP and high CTX in hip fracture were 25.38%,14.46%,36.27%,21.21%,and 25.17%,respectively,which were significantly higher than those in patients with spi-nal fractures(8.37%,7.07%,18.47%,10.42%,and17.21%).However,the proportion of patients with high ALP in hip fractures was significantly lower than that in patients with spinal fractures(28.87%vs.45.99%).Further compar-isons revealed that patients with hip fracture had significantly lower levels of calcium,phosphorus and serum ALP than pa-tients with spinal fracture,while serum PTH,P1NP and CTX levels were significantly higher than those in patients with spinal fracture(P<0.05).Conclusion Significant differences in BMBMs between patients with fragility fractures in the spine and hip may reflect differences in the pathophysiologic mechanisms of fractures at different sites.关键词
