Logistic Regression Analysis of Risk Factors for Severe Postoperative Complications in Thyroid Surgery

Abstract
Objective To assess and explore the risk factors for severe postoperative complications follow-ing thyroid surgery through logistic regression analysis.Methods A retrospective study was conducted on 60 patients who underwent thyroid surgery at the Second Affiliated Hospital of Fuzhou University from September 2021 to December 2024.Based on the presence or absence of severe postoperative complications,patients were divided into two groups:the severe complications group(n=16)and the non-severe complications group(n=44).The differences in age,sex,type of thyroid disease,type of surgery,neck lymph node dissection,surgery duration,comorbidities,intraoperative blood loss,and history of hyperthyroidism were compared between the two groups.Univariate and multivariate logistic regression analyses were performed to identify independent risk factors for severe postoperative complications.Results Among the 60 patients,16 developed severe postoperative compli-cations,with an incidence rate of 26.67%.The complications included 4 cases of asphyxiation(6.67%),3 cases of thyroid storm(5.00%),2 cases of massive bleeding(3.33%),and 7 cases of symptomatic hypocalcemia(11.67%).Univariate analysis revealed that,compared to the non-severe complications group,the severe complications group had significantly higher proportions of patients aged ≥ 60 years(31.25%vs.9.09%,P=0.034),total thyroidectomy(68.75%vs.25.00%,P=0.002),surgery duration ≥ 120 minutes(43.75%vs.18.18%,P=0.043),comorbid diabetes(31.25%vs.6.82%,P=0.014),and a history of hyperthyroidism(56.25%vs.18.18%,P=0.004).Multivariate logis-tic regression analysis identified that age ≥ 60 years,total thyroidectomy,comorbid diabetes,and a history of hyperthyroidism were independent risk factors for severe postoperative complications following thyroid surgery.Conclusion Age ≥ 60 years,total thyroidectomy,comorbid diabetes,and a history of hyperthyroidism are independent risk factors for severe postoperative complications following thyroid surgery.关键词
