Abstract
Objective To investigate the predictive value of serum nucleotide-binding oligomerization domain-like receptor family caspase activation and recruitment domain protein 4(NLRC4)for the prognosis of patients with acute supratentorial intracerebral hemorrhage(ICH).Methods The study retrospectively selected 87 patients with acute supratentorial ICH as the study subjects who were admitted to the Affiliated Hangzhou First People's Hospital,School of Medicine,Westlake University within 24 hours after onset and received conservative treatment between August 2020 and May 2023.The serum NLRC4 level at admission was measured.The risk of early neurological deterioration(END)was determined based on an increase of ≥4 points in the National Institutes of Health Stroke Scale(NIHSS)score or death within 24 hours after admission.Poor prognosis was defined based on a modified Rankin Scale score of 3-6 points at 90 days after onset.Multivariate logistic regression analysis was used to analyze the influencing factors for END risk and poor prognosis at 90 days after onset in acute supratentorial ICH patients.ROC curve analysis was used to evaluate the predictive efficacy of serum NLRC4 levels at admission for END risk and poor prognosis at 90 days after onset.Results NIHSS score at admission(OR=1.301,1.306,respectively),hematoma volume(OR=1.223,1.231,respectively),and serum NLRC4 level(OR=1.527,1.847,respectively)were all independent influencing factors for END risk and poor prognosis at 90 days after onset in ICH patients(all P<0.05).The AUC and 95%CI of serum NLRC4 level at admission for predicting END risk and poor prognosis at 90 days after onset in acute supratentorial ICH patients were 0.824(0.684-0.879)and 0.869(0.725-0.902),respectively,with cut-off values of 376.8 and 346.2 pg/mL,sensitivities of 0.712 and 0.677,and specificities of 0.826 and 0.911,respectively.Conclusion The serum NLRC4 level at admission has a high predictive value for the risk of END and prognosis at 90 days after onset in patients with acute supratentorial ICH.关键词
