单胺氧化酶对颈动脉粥样硬化早期诊断、斑块不稳定预测的价值研究
Value of monoamine oxidase in early diagnosis of carotid atherosclerosis and plaque instability prediction

Abstract
Objective To investigate the value of serum monoamine oxidase(MAO)levels in the early diagnosis of carotid atherosclerosis and in predicting plaque instability across different age groups.Methods A retrospective cohort was assembled from 327 patients diagnosed with carotid atherosclerosis at Dazhou Hospital Affiliated to North Sichuan Medical College between February 3,2022 and May 21,2023.Patients were stratified by age into 61 young,164 middle-aged,and 102 elderly individuals.According to carotid ultrasound findings,172 patients had unstable plaques and 155 had stable plaques.Eighty contemporaneous healthy subjects served as controls.MAO levels were compared among the unstable-plaque,stable-plaque,and control groups.Factors influencing plaque instability of patients with carotid atherosclerosis were analyzed.MAO levels were further compared across the three age groups.Correlations between MAO levels and intima media thickness(IMT)or plaque score of patients were examined.The predictive value of MAO for plaque instability within each age group was also assessed.Results The serum MAO levels were highest in the unstable-plaque group,followed by stable-plaque group,and the lowest in the control group(all P<0.05).Age(middle-aged and elderly),hypertension,TC,LDL-C,HDL-C,MAO,IMT,and plaque score were all influencing factors for plaque instability in patients(all P<0.05).Among unstable-plaque patients,MAO differed significantly among elderly,middle-aged,and young groups[(6.82±1.18)U/L vs.(5.51±0.89)U/L vs.(3.15±0.51)U/L,P<0.05];the same pattern was observed in stable-plaque patients[(5.07±1.01)U/L vs.(4.34±0.64)U/L vs.(3.07±0.59)U/L,P<0.05].In both contexts,MAO levels in different age groups were:elderly>middle age>young(all P<0.05).MAO level did not correlate with IMT in young patients(r=0.186,P>0.05),showed a weak positive correlation in middle-aged patients(r=0.190,P<0.05),and a strong positive correlation in elderly patients(r=0.704,P<0.05).In young patients,MAO level correlated moderately with plaque score(r=0.589,P<0.05)and a strong positive correlation was observed in both middle-aged(r=0.728,P<0.05)and elderly patients(r=0.816,P<0.05).The optimal MAO cut-off value for predicting plaque instability was 3.15 U/L(AUC=0.669)in young patients,3.79 U/L(AUC=0.735)in middle-aged patients,and 5.21 U/L(AUC=0.867)in elderly patients.Conclusion The association between MAO level and atherosclerosis strengthens with age.Particularly in middle-aged and elderly individuals,MAO can assess the severity of carotid atherosclerosis and predict plaque instability to certain degree.关键词
