宋耀鸿运用百合地黄汤治疗围绝经期心悸的临床经验
SONG Yaohong's clinical experience in treating perimenopausal palpitation with the Baihe Dihuang decoction

Abstract
Perimenopausal women's ovarian function decline,estrogen level decline,and autonomic nervous system dysfunction often lead to a series of symptoms,which is called perimenopausal syndrome in Western medicine.Cardiovascular symptoms in the patients are often more prominent.Studies have shown that downregulation of hormone levels not only destroys the electrophysiological balance of the heart,but also has many effects on the overall metabolism.Western medicine treatment is limited to hormone supplementation or symptomatic treatment,and fails to take into account many neuropsychological symptoms of patients.Combining the physiological and pathological basis of perimenopausal women,Professor SONG Yaohong focuses on adjusting the symptoms of palpitation,examining the pathogenesis,differentiating the syndromes and seeking the causes,tracing its origin to Huangdi Neijing(《黄帝内经》),and proposes that the pathogenesis of palpitation in perimenopausal women is Yin(阴)deficiency and it centers on the heart and kidney.Perimenopausal women face menstruation exhaustion,gradually deficient kidney water.Heart and kidney are dependent on each other,the imbalance of water and fire means that the heart Yin is weak,and the heart is uneasy,which leads to palpitation.At the same time,due to the lack of generating and transforming source of spleen and stomach Qi(气)and blood,or the heart failing of nourishment,the insufficiency of liver Qi causing disorder of Qi and blood of the whole body,palpitation and liver and spleen are also inseparable.Professor SONG believes that the disharmony between Yin and Yang(阳)is the premise of the occurrence of heart palpitation and is also the root cause.Therefore,its treatment should be based on nourishing Yin liquid,clearing deficiency heat,and paying attention to harmonizing liver and spleen,and balancing Yin and Yang.The classic prescription the Baihe Dihuang decoction(百合地黄汤)is composed of Baihe(Bulbus Lilii)and Dihuang(Radix Rehmanniae),which is originally designed for the syndrome of cardiopulmonary Yin deficiency and internal heat.Professor SONG clinically makes good use of the basic prescription for the treatment of perimenopausal palpitation,clearing the heart and calming the mind with Baihe,and cooling blood and nourishing Yin with Dihuang,which coincides with the pathogenesis of the disease.For patients with both heart and spleen deficiency,spleen deficiency and phlegm,and Yin and Yang deficiency syndrome,the prescription is modified flexibly,which is effective.One test case is attached.关键词
