The effects of macrophage polarization and mitochondrial homeostasis on myocardial ischemia-reperfusion injury based on the Yin-Yang theory in traditional Chinese medicine

Abstract
Myocardial ischemia-reperfusion injury(MIRI)is a common injury during the treatment course of ischemic heart dis-ease.In recent years,macrophage polarization and mitochondrial homeostasis have gradually become research hotspots for revealing the pathological mechanism of MIRI.As an important member of the immune system,macrophage can not only alleviate myocardial injury by secreting anti-inflammatory factors,but also may release a large number of pro-inflammatory response factors due to its excessive acti-vation,thereby intensifying myocardial inflammatory responses and further damaging myocardial tissue.Mitochondria are the core organs of energy metabolism in myocardial cells,and their balanced homeostasis is crucial for maintaining normal functions of myocardial cells.Mitochondrial autophagy is an important link in the regulation of mitochondrial homeostasis,and its abnormal activation or inhibition may cause metabolic disorders of myocardial cells and further aggravate the process of MIRI.Notably,macrophage polarization can play a bidi-rectional regulatory role in MIRI,and the homeostasis of mitochondria is affected by mitochondrial autophagy,further leading to metabolic abnormalities of myocardial cells.The polarization state of macrophages and the process of mitochondrial regulating its own homeosta-sis both exhibit dynamic processes.In view of this point,in this study,the theory of Yin-Yang(阴阳)in traditional Chinese medicine is innovatively introduced to explore the mechanism of macrophage polarization and mitochondrial homeostasis in the treatment of MIRI.The theory of Yin-Yang in traditional Chinese medicine emphasizes the dynamic balance and mutual restraint within the body,providing a new perspective for understanding the complex pathological process of MIRI.Restoring Yin-Yang balance by regulating macrophage polarization and mitochondrial homeostasis is expected to provide new strategies and ideas for alleviating MIRI and improving myocardial function.This finding not only enriches the pathological understanding of MIRI,but also opens up a new research path for the treatment of cardiovascular diseases in the integrative medicine.关键词
