Abstract
Diabetic nephropathy(DN)affects nearly 40%of diabetes patients and often leads to end-stage renal disease,necessitating renal replacement therapies such as dialysis and transplantation.The pathogenesis of DN is complex.Long-term hyperglycemia can induce the expression of advanced glycation end products,cytokines,growth factors,and activate signaling pathways.This process also triggers the expression of inflammation,endoplasmic reticulum stress,oxidative stress,mitochondrial dysfunction,and autophagy-related genes.There is no specific intervention for DN.Currently,lifestyle adjustments such as controlling blood glucose,lowering blood pressure,and regulating blood lipids are the main measures for clinical treatment of DN.Dysbiosis of intestinal flora can lead to abnormalities in flora distribution and metabolite secretion,affecting the integrity of the intestinal barrier,ultimately resulting in pathological responses such as endotoxemia,oxidative stress,and chronic inflammation.The interaction between the gut and kidney has been shown to directly or indirectly impact human health.In recent years,the methods for treating DN have been continuously updated both domestically and internationally.Traditional Chinese medicine has demonstrated good efficacy on intestinal microecology to alleviate renal damage,thus possessing unique advantages in the prevention and treatment of DN.Based on current research on the intervention of TCM on intestinal flora imbalance to improve DN,this article reviews the research progress of TCM monomers and compounds,and emphasizes the importance of targeting the gut-kidney axis to regulate the dysbiosis of DN flora,metabolites,and related signaling pathways,with the aim of expanding clinical thinking.关键词
