The effect of electroacupuncture on autophagy in cerebral ischemia-reperfusion injury model

Abstract
Cerebral ischemia reperfusion injury(CIRI)is a common pathological process following acute cerebral infarction,which can lead to neuronal damage or death.Autophagy,as a critical intracellular metabolic regulatory mechanism,degrades damaged organelles and abnormal proteins via lysosomal pathways,exhibiting a"double-edged sword"effect on maintaining neuronal homeostasis.Mitochondrial autophagy,as a selective autophagy process targeting dysfunctional mitochondria,plays a central role in neuroprotection by maintaining mitochondrial quality and energy supply.Both of them play a vital role in maintaining nerve corpuscle survival after CIRI.This study mainly explores the effects of electroacupuncture therapy on autophagy in nerve cells and mitochondrial autophagy in CIRI models.Studies have revealed that electroacupuncture bidirectionally regulates autophagy-related protein expression to modulate autophagy pathways,thereby protecting nerve cells.Electroacupuncture reduces autophagy level of nerve cells to preserve viable neurons in the initial stage of CIRI,and may protecting brain function by enhance autophagy activity to eliminate severely damaged neurons in the late phase.Furthermore,electroacupuncture may regulate autophagy through the AMPK-mTOR-ULK1 and PI3K-Akt-mTOR pathways,promote mitochondrial autophagy via the PINK1-Parkin and BNIP3L/NIX pathways,and clear damaged mitochondria,thereby improving mitochondrial quality.This study attempts to deeply explore the molecular mechanisms and spatio-temporal specificity effects of electroacupuncture.Notably,the bidirectional regulatory effects of electroacupuncture discovered in existing studies may correlate with acupoint selection strategies,aligning with the traditional acupuncture theory of"harmonizing Yin(阴)and Yang(阳)"highly.The temporal strategy of"suppressing autophagy to protect cells"in early stage and"enhancing autophagy to clear damage"in later stages may reflect the therapeutic wisdom of dynamically balancing the autophagy-mediated"survival-death"threshold.These findings provide scientific evidence for the application of electroacupuncture in treating CIRI and establish a foundation for further clinical research.关键词
