基于数据挖掘探讨李光善教授治疗糖尿病周围神经病变的用药规律
An analysis of the medication pattern of Professor LI Guangshan in treating diabetic peripheral neuropathy based on data mining

Abstract
Objective:To explore Professor LI Guangshan's therapeutic thoughts and medication patterns for diabetic peripheral neuropathy(DPN)based on data mining,providing a reference for the clinical practice of treating DPN.Methods:The outpatient medical records of treating DPN by Professor LI Guangshan from January 2022 to January 2024 were collected and sorted out,and were strictly screened out in accordance with the inclusion and exclusion criteria.The Excel 2016 software was used to established a database for an analysis of the frequency of Chinese medicine.The association rule analysis,clustering analysis,and factor analysis were carried out by IBM SPSS Modeler 18.0,RStudio,and IBM SPSS Statistics 26.0,respectively.Results:A total of 141 prescriptions were incorporated,involving 168 Chinese medicines.Among them,29 Chinese medicines had a frequency of more than 20 times,including Danggui(Radix Angelicae Sinensis),Baishao(Radix Paeoniae Alba),and Gancao(Radix Glycyrrhizae).Classified by the efficacy of Chinese medicines,tonifying medicines were mainly used.The medicinal properties were mostly warm.Sweet flavor medicines were frequently used,followed by bitter flavor medicines and pungent flavor medicines.The meridians involved were mainly spleen meridian,liver meridian,and kidney meridian.Through association rule analysis,86 associated combinations were obtained,mainly including Huangbo(Cortex Phellodendri Chinsis)-Cangzhu(Rhizoma Atractylodis)-Niuxi(Radix Achyranthis Bidentatae),Chishao(Radix Paeoniae Rubra)-Taoren(Semen Persicae)-Wugong(Scolopendra),and Shengdihuang(Radix Rehmanniae)-Maidong(Radix ophiopogon),etc.The clustering analysis identified 5 groups of medicine combinations with a relationship.The factor analysis identified 10 high-frequency medicine factors.Conclusion:Professor LI Guangshan believes that the pathogenesis of DPN is rooted in deficiencies of the liver,spleen,and kidneys,with manifestations of blood stasis and phlegm-dampness.The condition is characterized by a combination of deficiency in origin and excess in manifestation,involving both deficiency and excess simultaneously.The professor also points out that DPN typically begins with liver Yin(阴)deficiency,and the depletion of Yin affects Yang(阳)over time,leading to both spleen and kidney Yang deficiency.This progression results in blood stasis and phlegm-dampness obstructing the meridians,ultimately causing symptoms such as numbness and pain.In terms of treatment,emphasis is placed on the coordinated regulation of the liver,spleen,and kidney.Therapeutic principles include using Chinese medicines for balancing cold and warm,prioritizing supplementation,and treating both root and branch.According to syndrome differentiation,strategies such as benefiting Qi(气)and nourishing Yin,warming Yang and unblocking the collaterals,invigorating blood and resolving stasis,and clearing heat and drying dampness are applied,with the ultimate goal of restoring Yin-Yang balance and harmony of Qi and blood.关键词
