Abstract
With the change in social and demographic structure and the continuous rise in the prevalence of metabolic disorders,the disease burden of chronic heart failure has shown a significant growth trend,which has formed continuous pressure on the allocation of public health resources.Western medicine shows obvious limitations on this disease,such as adverse reactions and drug resistance.In recent years,based on the therapeutic features of holistic regulation and syndrome differentiation and treatment,traditional Chinese medicine has played an important role in alleviating the pain of patients,ameliorating clinical symptoms and long-term prognosis,and enhancing patients'quality of life.In this study,starting from the theory of"only when the dominant role of the sovereign fire is normal can the role of the ministerial fire be normal"in Huangdi Neijing(《黄帝内经》),by sorting out the etiology of chronic heart failure,it is concluded that the occurrence of this disease is related to prolonged illness,old age,and frailty due to Yang-qi(阳气)damage of the human body,and that the basic pathogenesis of this disease is"the sovereign fire is disordered and the ministerial fire is not in position".Sovereign fire of the heart and ministerial fire of the kidneys are deficient and unable to support each other,and heart Qi is unable to circulate normally,producing blood stasis and retained fluid.Therefore,a three-level treatment system of warming heart Yang,tonifying kidney Yang and resolving blood stasis is constructed based on this pathogenesis:Warming heart Yang to enhance the clarity of the sovereign fire,tonifying kidney Yang to restore the position of the ministerial fire,and resolving blood stasis and promoting diuresis to treat the symptoms,thereby establishing a comprehensive treatment system of harmonizing the heart and kidneys,treating from internal organs,and taking into account both the symptoms and the root causes.In this study,the significance of the theory of"only when the dominant role of the sovereign fire is normal can the role of the ministerial fire be normal"in the identification and treatment of heart failure is explored from the pathogenesis and treatment of heart failure,providing a reference for the diagnosis and treatment of chronic heart failure in traditional Chinese medicine.关键词
