基于五运六气理论辨治肾病综合征
The differentiation and treatment of nephrotic syndrome based on the theory of five evolutive phases and six climatic factors

Abstract
Nephrotic syndrome is a clinical syndrome primarily characterized by massive proteinuria,hypoalbuminemia,edema,and hyperlipidemia.It is a relatively common kidney disease in clinical practice,with rather complex etiology and pathogenesis.Western medicine offers limited treatment options for nephrotic syndrome,and the patients may face high risks of infection or other adverse reactions.Traditional Chinese medicine has unique advantages in the prevention and treatment of nephrotic syndrome.The theory of five evolutive phases and six climatic factors originates from the classic work of Huangdi Neijing(《黄帝内经》),and is an important component of traditional Chinese medical theory.Based on the theory of Celestial Stems and Terrestrial Branches and the generation and regulation correlation between five evolutive phases and six climatic factors,the relationship between climate changes and diseases is deduced and predicted.Guided by the core ideology of"harmony between heaven and humanity",the role of natural climate on human physiology and pathology is explored.In this study,the diagnostic and therapeutic thoughts for nephrotic syndrome are analyzed based on the theory of five evolutive phases and six climatic factors.Based on the theory of five evolutive phases and six climatic factors,the specific spatiotemporal conditions,climate states,and the onset characteristics and treatment patterns of nephrotic syndrome patients with different TCM constitutions are analyzed.Chief physician ZHOU Hongquan has gained considerable insights into diagnosing and treating nephrotic syndrome based on the theory of five evolutive phases and six climatic factors,and believes that the key to differentiating and treating the disease lies in"observing symptoms and grasping pathogenesis",emphasizing the changes of Qi(气)and blood and Qi circulation in human body due to the transformation and restricting excessive Qi to restore harmony in the body caused by the five evolutive phases and six climatic factors.Additionally,by taking into account the differences in innate endowments brought about by the five evolutive phases and six climatic factors at birth,as well as the patient's pulse and syndrome symptoms,a comprehensive analysis and syndrome differentiation was conducted based on the principles of governing heaven,governing human,and governing disease and syndrome.By consulting Sanyin Sitian Fang(《三因司天方》)and integrating the theory of opening-closing pivot gasification,classic prescriptions are flexibly used to treat nephrotic syndrome,achieving good results,and providing ideas for the treatment of nephrotic syndrome in traditional Chinese medicine.关键词
