Abstract
Hepatorenal syndrome(HRS)is a severe complication in patients with decompensated liver cirrhosis.It is a functional renal failure that occurs on the basis of liver cirrhosis plus ascites.Once diagnosed,prompt initiation of treatment is crucial to prevent renal function failure.Currently,the common treatment methods include terlipressin plus human serum albumin injection,diuretics(tolavtan),artificial liver and renal replacement therapy,liver transplantation,etc.This disease is difficult to treat,with a low overall survival rate and poor prognosis of patients.In traditional Chinese medicine,HRS belongs to the categories of"tympanites","edema",and"block and repulsion".Its clinical manifestations include dull complexion,poor appetite and weakness,abdominal fullness,abdominal distension,lower limb edema,yellowing of eyes and urine,and urinary obstruction.In the treatment of this disease,Professor SUN Wei started from the theory of triple energizers,combined location triple energizer with Zang-fu organs(脏腑)triple energizers,and believed that the pathogenesis of HRS was deficiency in origin and excess in superficiality,taking the deficiency of the liver,spleen,and kidney as the root cause,and Qi(气),blood,phlegm,dampness,and blood stasis as the superficial symptoms.Additionally,the changes such as pathogenic factors invading the heart spirit,and collaterals damage and blood overflow may occur in the later stage of the disease.In view of the relationship between the theory of triple energizers and HRS,Professor SUN Wei proposed the treatment method of"promoting upper energizer,strengthening the transportation and transformation functions of middle energizer,warming lower energizer,and preventing obstruction of triple energizers",and the treatment principle of"taking into account treating both superficial symptoms and root causes,treating the symptoms in urgent cases,and treating the root cause in chronic cases".In this study,Professor SUN Wei's guiding ideology in explaining the etiology,pathogenesis,and syndrome differentiation and treatment of HRS based on the theory of triple energizers,and the experience of prescription and medication are summarized.Meanwhile,one empirical case is analyzed and discussed,in order to provide more differential medication ideas for the treatment of HRS.关键词
