经内科胸腔镜下胸膜粘连烙断术在结核性包裹性胸膜炎中的临床应用
Clinical application of cauterization of pleural adhesion by medical thoracosco-py in the treatment of tuberculous encapsulated pleurisy

Abstract
Objective To investigate the clinical value of cauterization of pleural adhesion by medical thoracos-copy in the treatment of encapsulated tuberculous pleurisy.Methods The clinical data and microscopic manifesta-tions of patients with tuberculous pleurisy who underwent medical thoracoscopy in the tuberculosis area at the De-partment of Respiratory and Critical Care Medicine,Affiliated Hospital of Zunyi Medical University,from January to September 2024 were retrospectively analyzed.According to the different microscopic manifestations,the pa-tients were divided into non-encapsulated adhesion group(control group),mild adhesion group,moderate adhe-sion group and severe adhesion group.The course of disease,intraoperative drainage volume,postoperative drainage volume and recovery at 6 months after operation were compared and analyzed among the four groups.Results Among 128 patients with tuberculous pleurisy,there were 44 cases of non-encapsulated adhesion,17 ca-ses of mild adhesion,32 cases of moderate adhesion and 35 cases of severe adhesion.There was no significant difference in gender,age and drainage days among the four groups(P＞0.05).There were significant differ-ences among the four groups in the course of disease,intraoperative drainage volume,postoperative drainage vol-ume,and recovery at 6 months after surgery(P＜0.05).The course of disease in the severe adhesion group was longer than that in the non-encapsnlated group and mild-to-moderate adhesion groups.The good recovery rate of chest CT at 6 months after operation was 100％,94.12％,93.75％and 54.29％,respectively.The rates of patients with pleural thickening were 0％,5.88％,6.25％and 2.85％,respectively.There were pleural thick-ening and a small amount of encapsulated effusion,partial atelectasis at 0％,0％,0％,42.86％,respectively.No obvious complications occurred in all patients.Conclusion Patients with tuberculous pleurisy should be trea-ted as soon as possible,and effective treatment measures should be taken in time to promote recovery and reduce sequelae.Cauterization of pleural adhesion under medical thoracoscopy has important clinical application value in the local treatment of tuberculous encapsulated pleurisy with mild to moderate adhesion.关键词
