中西医结合多学科的直肠癌保肛术后康复方案对脾虚型低位前切除综合征患者的临床疗效观察

Abstract
Objectives To observe the therapeutic effect of an integrated traditional Chinese and Western medicine(TCM-WM)multidisciplinary rehabilitation program in improving defecation dysfunction symptoms and quality of life(QoL)in patients with spleen deficiency-type low anterior resection syndrome(LARS)after sphincter-preserving surgery for rectal cancer.Methods A total of 51 patients with spleen deficiency-type LARS after sphincter-preserving surgery for rectal can-cer were enrolled from colorectal clinic and the multidisciplinary rectal cancer rehabilitation clinic at Xiyuan Hospital,China Academy of Chinese Medical Sciences,between May 2023 and December 2024.Based on patient preference,44 pa-tients were included for final data analysis and divided into a control group(n=22,receiving conventional TCM treat-ment)and an intervention group(n=22,receiving the integrated TCM-WM multidisciplinary rehabilitation program).Daily bowel movement frequency was recorded.LARS score,defecation-related symptom score,defecation satisfaction score,and QoL score were assessed before and 90 days after treatment.Results Before and after treatment,no statistically signifi-cant differences were observed between the two groups in daily bowel movement frequency,LARS score,defecation-related symptom score,defecation satisfaction score,or QoL score(P＞0.05).In the intervention group,significant differ-ences were found after treatment in daily bowel movement frequency,LARS score,and defecation satisfaction score com-pared to those before treatment(P＜0.05).In the control group,significant differences were observed in the LARS score and defecation satisfaction score after treatment compared to those before treatment(P＜0.05).After adjusting for base-line values,analysis of covariance showed that the intervention group had significantly better outcomes in daily bowel movement frequency,LARS score,defecation-related symptom score,and defecation satisfaction score compared to the con-trol group(P＜0.05),but no significant difference in QoL score(P＞0.05).Conclusion The integrated TCM-WM multidis-ciplinary rehabilitation program,combining herbal medicine,pelvic floor exercise,bowel habit training,and nutritional sup-port,improved defecation in patients with spleen deficiency-type LARS,which has a good development prospect.关键词
