基于人工智能的宫颈癌筛查系统在宫颈上皮内瘤变诊断中的准确性评估
Accuracy evaluation of AI-based cervical cancer screening system in the diagnosis of cervical intraepithelial neoplasia

Abstract
Objective:To evaluate the diagnostic accuracy of an AI-based cervical cancer early screening system(TruScreen)for cervical intraepithelial neoplasia(CIN).Methods:156 patients with suspected CIN admitted to the First Affiliated Hospital of Air Force Medical University from June 2022 to October 2024 were selected.Using histopathological results as the"gold standard",the findings of histopathology,colposcopy,and TruScreen were recorded.Receiver operating characteristic(ROC)curves were plotted to analyze the diagnostic efficacy of colposcopy and TruScreen for CIN,and their consistency with histopathological results was evaluated.Results:Histopathological examination showed 109 CIN-negative cases(69.87%)and 47 CIN-positive cases(30.13%).Colposcopy indicated 115 CIN-negative cases(73.71%)and 41 CIN-positive cases(26.29%).TruScreen screening revealed 110 CIN-negative cases(70.51%)and 46 CIN-positive cases(29.49%).ROC curve analysis suggested that the area under the curve(AUC)for colposcopy and TruScreen in diagnosing CIN was 0.738(95%CI:0.645-0.831,P<0.001)and 0.867(95%CI:0.795-0.939,P<0.001),respectively.The sensitivity and specificity were 59.57%and 88.07%for colposcopy,and 90.85%and 92.66%for TruScreen.Kappa consistency tests indicated good agreement between both methods and histopathology,with Kappa coefficients of 0.633 for colposcopy and 0.715 for TruScreen.Conclusion:Both colposcopy and TruScreen have certain diagnostic value for CIN,but TruScreen shows higher diagnostic efficacy,making it more suitable for clinical screening of CIN and worthy of clinical promotion.关键词
