机器人辅助腹腔镜肾部分切除术在老年肾癌患者中的临床疗效分析
Clinical efficacy of robot-assisted laparoscopic partial nephrectomy in elderly patients with renal cell carcinoma

Abstract
Objective:To explore the clinical efficacy of robot-assisted laparoscopic partial nephrectomy(RALPN)in elderly patients with renal cell carcinoma.Methods:116 elderly patients with renal cell carcinoma who were treated at the Drum Tower Hospital Affiliated to Nanjing University Medical College From January 2021 to January 2024 were selected and randomly divided into the control group(n=58)and the observation group(n=58).The control group underwent conventional laparoscopic partial nephrectomy,and the observation group underwent RALPN.Perioperative parameters,pre-and post-operative renal function markers,and complication rates were compared between the two groups of patients.Results:The observation group showed significantly shorter operative time,warm ischemia time,time to oral intake,and length of hospital stay compared to the control group(P<0.05),alongside reduced intraoperative blood loss and postoperative drainage volume(P<0.05).One month after surgery,both groups showed elevated SCr and BUN levels and decreased eGFR levels compared to those before surgery(P<0.05).However,the observation group had significantly lower SCr and BUN levels and higher eGFR levels than the control group(P<0.05).The total complication rate(including incision infection,postoperative hemorrhage,stress ulcers,and intestinal obstruction)in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:RALPN has advantages of reduced operative time,minimized surgical trauma,faster postoperative recovery,enhanced renal function preservation,and fewer complications in treating elderly patients with renal cell carcinoma,which has good clinical application value.关键词
